Exploring a new approach to improving health, lifestyle and social care
About the study
In the UK, many people are living with two or more long term health conditions (including mental health conditions). People with these conditions often need further lifestyle and social support.
We want to improve this support and ask for your views on a potential new approach to care delivery. This approach uses NHS data to group together people who might have similar health, lifestyle and social needs. We call this group a ‘cluster’.
Using this cluster approach, we hope to improve care delivery. Before we think about using this approach, it's important to find out what people think about this idea.
What will it involve?
If you would like to take part, we will ask you to:
- Read and sign a consent form
- Complete some questions about yourself in a survey
- Have a chat with a researcher for up to 60 minutes. This can be online or on the telephone.
